Prepare the v1.2.1 patch release changelog.

Includes the Agent updates UI corrections from #50:

  • no typed-count confirmation
  • working name, version, and eligibility filters
  • no polling or DOM replacement while selecting hosts
  • progress polling only during an active rollout
